Position Details:

Join our team as an Embedded Nurse Practitioner or Physician Assistant! In this impactful role, you'll conduct annual wellness exams and vital screenings that enhance the quality of patient care. You'll perform thorough health assessments, conduct non-invasive screenings, and offer personalized recommendations for disease management. Your contributions to patient education and health promotion will make a real difference in our community!

Role: Open to Nurse Practitioners and Physician Assistants

Location: Rotation between Storrs, Windham, Vernon, Windsor and South Windsor

Setting: 100% Outpatient Primary Care

Schedule:  Full-time, weekdays only. Ask about our flexible schedule options!

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.


Requirements:

·         Licensure: Unrestricted State of Connecticut Medical License (or eligible)

·         Training: Nurse Practitioner (Family, Adult/Geri Nurse Practitioner) or Physician Assistant

·         Experience: 2+ years’ experience in a primary care or relevant outpatient setting
